How to access our Patient Online service

Our online service provides greater choice and flexibility in accessing some of our services from your computer, tablet or mobile phone.

You will be able to:

You will need to register to use our online services separately to registering with us as a patient.

How to register to use our online services

STEP ONE

Tell our reception staff that you want to start using the online service. They will give you a short form to complete. Alternatively you can download the form here and bring completed to the surgery.

Remember you will need to bring photo proof of ID and a proof of address (i.e. utility bill) with your completed registration form. If you do not have any ID then either a member of staff will have to confirm your identity or you may have to answer questions about personal information on your GP record.

STEP TWO

We will process your registration request and issue you with a letter in the post containing your unique username and password. Please keep this safe and do not share with anyone else.
